• Copy of Birth Certificate or Passport (For New Players to CUFC CLASSIC and ALL Pre-Academy/Academy players)
• 1" x 1" Color Photo with Name Printed on Back (Face Only - No Black & White)
• Non-Refundable Payment. Bring your receipt for online payment or a check. PAY DEPOSIT & FEES ONLINE!
• If you DO NOT LIVE in North Carolina, completed Region III Release Form (the last page in the Player Registration packet). NOTE: If residing in South Carolina, please bring this form along with a $10 check made payable to SCYSA. - ACADEMY & CLASSIC Only!
